A plan, not just appointments

Long-term conditions do best with structure. Instead of reacting each time something flares, a GP Management Plan sets out your goals, the tests and reviews you need through the year, and who else is involved in your care. It also unlocks Medicare-subsidised allied health visits, which is how many patients first get to see a physiotherapist, podiatrist or dietitian without a large out-of-pocket cost.

Your GP prepares the plan with you, our nurse helps track the reviews, and everything stays in one record at one clinic.

Conditions we manage

Our GPs regularly look after:

  • Type 2 diabetes and pre-diabetes, including HbA1c monitoring and cycle-of-care checks
  • High blood pressure, high cholesterol and cardiovascular disease
  • Asthma and COPD, with spirometry referral and action plans
  • Arthritis and other musculoskeletal conditions
  • Chronic pain
  • Thyroid, kidney and liver conditions
  • Ongoing mental health conditions

The team around you

Team Care Arrangements let your GP bring allied health into the plan. Chiropractic, physiotherapy, podiatry and dietetics all operate from this building, and 4Cyte Pathology collects on site, so a review that used to mean three separate trips can often be done in one.

If you have been diagnosed with a long-term condition and have never had a care plan prepared, book a long appointment and ask about one. It is bulk billed for eligible patients.
